We've Moved!

1/7/2016

After over 25 years at our Foothill and Fruitvale location, the Health Care for the Homeless Program will be moving to new offices at 14th and Franklin Street in downtown Oakland on January 11, 2016.

Please take note of our new contact information below:

Alameda County Health Care for the Homeless Program
1404 Franklin St., Suite 200
Oakland, CA 94612
510-891-8950

Staff serving clients will be located in the same building on the first floor, but the entrance is around the corner:

384 14th St.
Oakland, CA 94612
510-891-8950

A few of our administrative staff will be located at the Health Care Services Agency Offices in San Leandro:

1000 San Leandro Blvd., Suite 300
San Leandro, CA 94577
510-667-4487
From 1988 to the present, dozens of dedicated HCH staff members worked out of our Fruitvale offices, providing health care and support services to over 120,000 Alameda County residents experiencing homelessness. Our new location will enable us to collaborate more closely with our health care and housing partners as we continue our proud tradition of compassionate service. We anticipate hosting an event in Spring 2016 to introduce the new location to our partners and share more about the great things happening in Health Care for the Homeless.

It is important to note several changes to our services before then. We will no longer provide TB testing on Tuesday and Thursday afternoons at the new location. We have already reached out to several of you to explore the possibility of providing testing at community sites when our nurse practitioner returns from leave in mid-February, and we’ll let you know more about our plans for TB testing as that date approaches.

Our Officer of the Day service which was formerly available on Monday, Wednesday, and Friday mornings will also come to an end. Please call 211 for information and referral to community resources including emergency shelter.

Until our open house in Spring 2016, please feel free to call or email me with questions or concerns. Thanks for your support and partnership over the years.
