Respite and Care Management
As part of the HCH system of care, the Alameda Health System Care Transitions Program at Highland Hospital provides complex care management services to high-risk patients. Patients experiencing homelessness receive support from a team that includes nurses, doctors, social workers and community health workers for issues related to chronic disease management, substance abuse, homelessness, and self-care.
