Trust Health CenterThe mission of the ACHCH/LifeLong Trust Health Center is to improve the lives of clients by providing high-value care that addresses a clients’ physical, mental, and social well-being in one location. This collaboration between multiple government agencies and non-profit organizations will promote dignity, respect, and compassion in all interactions among staff and clients. Participating organizations will collaborate closely to support clients to find and maintain stable homes, to promote their overall health and well-being, and to develop or re-establish strong social support networks outside of the public services system.
Primary care at the Trust Health Center is offered by LifeLong Medical Care under contract with Alameda County Health Care for the Homeless (ACHCH). Our core services include:
