Trust Health Center

The mission of the ACHCH/LifeLong Trust Health Center is to improve the lives of clients by providing high-value care that addresses a clients’ physical, mental, and social well-being in one location. This collaboration between multiple government agencies and non-profit organizations will promote dignity, respect, and compassion in all interactions among staff and clients. Participating organizations will collaborate closely to support clients to find and maintain stable homes, to promote their overall health and well-being, and to develop or re-establish strong social support networks outside of the public services system. ​

Primary care at the Trust Health Center is offered by LifeLong Medical Care under contract with Alameda County Health Care for the Homeless (ACHCH).

  • Primary health care
  • STD, HIV, TB, and other lab testing
  • Comprehensive behavioral health care
    • Psychiatry
    • Psychotherapy
  • Recovery focused medical care
    • Suboxone (Buprenorphine) treatment
  • Specialty medical care
    • Acupuncture
    • Podiatry
  • Housing coordination/support
  • Health coach support​
  • Donated clothing and shoes
  • Computer and internet access
  • Group activities
    • Men's and Women's groups
    • Health and wellness groups
    • Mindfulness and art groups
    • Recovery focused groups
    • Housing workshop

​To become a client of the Trust Health Center a person must be:
  • An adult
  • A resident of Alameda County
  • Homeless or at-risk of homelessness OR receiving case management from Alameda County Behavioral Health

Trust Health Center’s diverse and unique care team supports health through encouragement, and by blending medical, mental and social care under one roof. We help people experiencing homelessness in the Bay Area through an intentionally created community where people without their own safe space feel welcomed and accepted.
