Trust Health Center
The Trust Health Center works to improve the lives of clients by providing high-value care that addresses physical, mental, and social well-being in one location. Partner organizations collaborate closely to support clients to find and maintain stable homes, to promote overall health and well-being, and to develop or re-establish strong social support networks outside of the public services system.
Primary care at the Trust Health Center is offered by LifeLong Medical Care under contract with Health Care for the Homeless.

The following organizations work with clients to the Trust Health Center: HCH Program, Homeless Action Center (HAC), Bay Area Legal Aid, Social Services Agency SSI Unit, Mental Health Advocates, Bay Area Community Services, or Oakland STOMP (Roots Community Health Center).

Trust Health Center’s diverse and unique care team supports health through encouragement, and by blending medical, mental and social care under one roof. We help people experiencing homelessness in the Bay Area through an intentionally created community where people without their own safe space feel welcomed and accepted.
